By signing or submitting this form, you agree to the following policies of Sonia’s Performing Arts @ Sawyer Station HTX Policy Agreement: 

Code of Conduct:

• I agree to treat all instructors, staff, and fellow students with respect and maintain a positive attitude during classes and studio events.

• I will adhere to the studio’s dress code, including wearing appropriate attire and clean shoes to protect the dance floor.

• I will follow safety guidelines, including reporting any injuries or concerns to the teacher/instructor immediately and refraining from dancing under the influence of alcohol or drugs.

• I will arrive on time for private lessons & group classes and notify the studio at least 24 hours in advance if I cannot attend a scheduled lesson.

Payment Terms:

• I agree to pay all tuition, fees, or lesson packages as outlined by the Sonia’s Performing Arts @ Sawyer Station HTX, either upfront or according to the agreed payment schedule.

• I understand that private lessons & group class payments are non-refundable, and private lesson cancellations require at least 24 hours’ notice to reschedule without penalty.

• Lesson packages must be used within 1 (One) year from the purchase date, and unused lessons will not be refunded.

• Late payments may incur a fee of $25, and I authorize the Sonia’s Performing Arts @ Sawyer Station HTX to charge my provided payment method for recurring fees if applicable.

No Refund Policy:

• I understand and agree that all payments for group classes, private lessons, workshops, or lesson packages are non-refundableunder any circumstances, including but not limited to missed classes, personal scheduling conflicts, or withdrawal from the program.

• Group class fees must be paid at least 24 hours in advance, and no refunds or credits will be issued for missed or unattended classes.

• Private lesson cancellations require at least 24 hours’ notice to reschedule without penalty; however, no refunds will be provided for canceled or unused lessons.

• Lesson packages must be used within 1 (One) Year from the purchase date, and any unused lessons will expire without refund or credit.

Liability Waiver

• Assumption of Risk: I acknowledge that ballroom dancing and related activities involve inherent physical risks, including but not limited to sprains, strains, falls, collisions, or other injuries. I voluntarily assume all risks associated with participating in private lessons, group classes, workshops, events, or other activities at Sonia’s Performing Arts @ Sawyer Station HTX, whether on studio premises or at off-site events organized by the Sonia’s Performing Arts @ Sawyer Station HTX. 

• Release of Liability: I hereby release, discharge, and agree to hold harmless Sonia’s Performing Arts @ Sawyer Station HTX, its owners, instructors, employees, contractors, and affiliates (collectively, “Released Parties”) from any and all liability, claims, demands, or causes of action for personal injury, property damage, or other losses sustained by me during or as a result of my participation in Sonia’s Performing Arts @ Sawyer Station HTX activities, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

• Medical Authorization: In the event of a medical emergency, I authorize Sonia’s Performing Arts @ Sawyer Station HTX to seek and obtain necessary medical treatment on my behalf. I agree to be responsible for any associated medical costs.

• Health Representation: I confirm that I am physically capable of participating in dance, and other related activities and have no medical conditions that would prevent safe participation, unless disclosed in writing to the Sonia’s Performing Arts @ Sawyer Station HTX prior to participation.

Media Consent:

• I grant  Sonia’s Performing Arts @ Sawyer Station HTX permission to use photographs or videos of me taken during private lessons, group classes or events for promotional purposes, including on the  Sonia’s Performing Arts @ Sawyer Station HTX’s website, social media, or marketing materials.

• I understand I can opt out of media use by notifying  Sonia’s Performing Arts @ Sawyer Station HTX in writing at the time of registration.

Additional Policies

• Class Registration: I agree to pre-register for group classes, camps, programs as required, understanding that class sizes may be limited and that missed classes cannot be made up unless explicitly offered by the Sonia’s Performing Arts @ Sawyer Station HTX

• Compliance: Failure to comply with these policies may result in suspension or termination of my participation in activities in Sonia’s Performing Arts @ Sawyer Station HTX without refund.

Arbitration Clause

• Agreement to Arbitrate: Any dispute, claim, or controversy arising out of or relating to this agreement, my participation in [Studio Name] activities, or any services provided by the studio, including but not limited to claims of negligence, breach of contract, or personal injury, shall be resolved exclusively through binding arbitration administered by the American Arbitration Association (AAA) under its Commercial Arbitration Rules.

• Arbitration Process: The arbitration will be conducted in [City, State] by a single arbitrator selected in accordance with AAA rules. Each party will bear their own legal fees and costs, and the studio will cover the arbitrator’s fees unless otherwise determined by the arbitrator. The arbitration award shall be final and binding.

Acknowledgment: By signing below or submitting this form, I confirm that I have read, understood, and agree to abide by the above liability waiver and policies, including the no refund policy and binding arbitration clause. I understand that these terms are a condition of my participation in Sonia’s Performing Arts @ Sawyer Station HTX activities.

By entering my name below, I acknowledge that I have read, understood, and agree to the Sonia’s Performing Arts @ Sawyer Station HTX Policy Agreement. I understand this constitutes a legally binding agreement.
